Which application should be completed?

The LCAF Application for Financial Assistance should be used for most circumstances. Please only use the Natural Disaster Application for large-scale natural disasters where many team members are impacted. (For example: flooding, hurricanes, earthquakes, etc.)

Where can I find the applications?

The LCAF Application for Financial Assistance and the Natural Disaster Application can be downloaded by clicking the “Emergency Hardship” and “Natural Disaster” links above.

How long does it typically take to receive a determination on my applications?

The Grant Panel meets monthly; however, the Grant Panel may meet on an emergency basis if there is an urgent need. You will be contacted by phone as soon as a decision is reached in your case.

Am I able to re-apply?

Yes. If an urgent need arises, you may re-apply and provide any new information for consideration but there are applicable limits in place to ensure equitable treatment to those in need.

Can I make a one-time donation?

Yes. You can make a one-time donation by clicking on the PayPal link above, signing up for a Payroll deduction, or sending a check to LCAF, c/o P.F. Chang’s China Bistro, Inc., 8377 E. Hartford Dr, Suite 200, Scottsdale AZ 85255. Please make all checks payable to “Lucky Cat Assistance Fund.”

Is my donation tax deductible?

Yes, your donation may be tax deductible as the Lucky Cat Assistance Fund is at tax-exempt, non-profit organization under Section 501(c)3 of the Internal Revenue Code. Click here for a copy of the Donation Receipt Tax Record Form. (For team members of P.F. Chang's China Bistro, Inc., you can find the total amount of your payroll donations on your paystub.)

How much financial support can I receive?

Grants amounts typically range between $500 to $2,000.

How many grants can I receive?

Grant recipients typically receive only one grant per year.

How can I keep track of my total donations?

The total amount of your annual donations can be found under the year-to-date deductions section on your pay statement or by logging in to PeopleXpress at ess.pfcb.com and clicking on Myself > Current Pay Statement.
